Refer a Patient to Dynamic Physical Therapy
One-on-one care from advanced-trained therapists. In-clinic electrodiagnostics and imaging. Coordinated reporting back to your practice. Across Queens & Long Island.
Three Ways to Send Us Your Patients - All Fast, All Friction-Free
Referring to Dynamic Physical Therapy is straightforward. We accept referrals by phone, fax, and walk-in. We verify insurance benefits before the first appointment and handle all intake - so the administrative burden stays with us, not your staff.
A prescription is not required in New York State for initial evaluation and treatment under Direct Access for physical therapy. However, many commercial plans and No-Fault carriers require a prescription for billing - we will confirm this with your patient's specific plan during insurance verification and advise accordingly.
Call Us Directly
The fastest route. Call (718) 826-3200 and our intake team will schedule your patient immediately, confirm insurance, and advise on any prescription requirements for their specific plan.
Fax Your Prescription / Referral
Send your prescription or referral slip to our intake team. Include the patient's name, date of birth, diagnosis, and any specific precautions. We will contact the patient directly to schedule and confirm coverage.
Have Your Patient Call Directly
Give your patient our number - (718) 826-3200 - and they can schedule their own intake. We will handle insurance verification, confirm whether a prescription is needed for their plan, and get them in quickly.
Complimentary Assessment First
For patients who are uncertain about proceeding with PT, we offer a free complimentary clinical assessment - a full evaluation with a licensed therapist, at no charge, that establishes the diagnosis and outlines the treatment plan before any financial commitment.

What Your Patients Receive When You Refer to Dynamic PT
The quality of a physical therapy referral is determined by what actually happens in the clinic. These are the clinical standards that make a Dynamic PT referral different from a generic one.
One-on-One with a Licensed Therapist - Every Session
Every session at Dynamic PT is conducted entirely by the licensed therapist - not by a technician or aide. Your patient's therapist performs the evaluation, delivers the treatment, and progresses the plan at every visit. The clinical relationship is direct and uninterrupted. This is not the standard at most PT clinics, and it is the most significant determinant of outcome quality.
Advanced Techniques Far Beyond Minimum Licensure
Our therapists are trained in manual therapy, spinal manipulation, neural mobilization, myofascial release, Kinesio taping, cupping, dry needling, and advanced neuromuscular retraining techniques. Dr. Nilesh Soni holds a GCS (Geriatric Clinical Specialist) designation - one of the APTA's advanced specialty certifications, reflecting the clinical standard our team maintains.
Progress Reports & Discharge Summaries Back to You
We communicate directly with referring physicians - providing evaluation findings, interim progress reports, and discharge summaries. For post-surgical patients, we review operative reports and confirm protocol with the surgeon before the first treatment session. Your patient's care does not happen in a black box.
In-Clinic Electrodiagnostics - No Separate Referral Needed
We perform EMG/NCS, evoked potentials (SSEP/VEP/BAER), musculoskeletal ultrasound, and VNG vestibular testing in our clinics. If a diagnostic question arises during your patient's PT course, it can be answered in the same visit - without a separate referral to a neurology lab or imaging centre. Results are available immediately and shared with your practice.
Multilingual Clinical Team
Our team speaks English, Hindi, Gujarati, Marathi, Urdu, and Punjabi. For patients in your practice who are most comfortable communicating in one of these languages, we can ensure their therapist shares that language - improving the accuracy of assessment and the effectiveness of education and home programming.
Insurance Verified Before the First Visit
We verify complete insurance benefits before every first appointment - confirming coverage, out-of-pocket costs, and any prescription or prior authorisation requirements. Your patient arrives knowing exactly what to expect financially. For No-Fault and Workers' Compensation cases, we handle all carrier coordination.
Electrodiagnostic & Imaging Studies Available In Our Clinics
Your patients can receive diagnostic testing and physical therapy in the same facility - often in the same visit. Results are generated in-clinic, not sent to an external laboratory, and are available for transmission to your practice the same day.
Electromyography & Nerve Conduction Studies (EMG/NCS)
Objective electrodiagnostic assessment of peripheral nerve and muscle function. Confirms and grades radiculopathy, carpal tunnel syndrome, peripheral neuropathy, plexopathy, and muscle disease. Up to 50% of initial diagnoses change after EMG/NCS evaluation. Covered by Medicare, Medicaid, No-Fault, and most commercial plans.
Evoked Potentials - SSEP, VEP & BAER
Non-invasive (no needles) tests assessing central sensory pathway integrity from peripheral nerve through spinal cord, brainstem, and cortex. Supports MS diagnosis, documents spinal cord compromise, screens for acoustic neuroma, and provides objective CNS pathway documentation for MVA cases.
Musculoskeletal Ultrasound (MSKUS)
Real-time dynamic imaging of tendons, muscles, ligaments, bursae, and nerves. No radiation. Equal to or superior to MRI for rotator cuff tears, Achilles tendinopathy, lateral epicondylitis, and nerve entrapment. Dynamic assessment reveals pathology only visible during movement - unavailable on static MRI.
Videonystagmography (VNG) - Vestibular Testing
Gold-standard vestibular function assessment using infrared video goggle recording of nystagmus. Three-component battery: oculomotor, positional, and caloric testing. Confirms BPPV (all canal types), vestibular neuritis, Meniere's disease, and unilateral hypofunction. Identifies central red flags requiring neurological referral.
All diagnostic studies are documented and transmitted to referring physicians. Learn more about our diagnostic services →

We Report Back. Your Patient's Rehabilitation Is Not a Black Box.
