Before your first visit, our intake team contacts your insurer and confirms your co-pay, deductible, authorised visits, and whether a prescription is required - then calls you with the results.
Download and Complete Your Forms
Download the relevant forms from our Patient Forms page and complete them before arriving. Bring your completed forms, insurance card, photo ID, and any physician prescription.
Arrive for Your Complimentary Assessment
