Send prescriptions, referral forms, operative reports for post-surgical patients, and records requests by fax. Include the patient's name, date of birth, diagnosis, and - for diagnostic testing orders - the specific study requested and its clinical indication. We confirm receipt of all physician faxes within one business day.

Referring physicians may request records for any active patient at any time during the course of care - no formal release process required. For records after discharge, a signed patient authorisation is required under HIPAA. Call our clinical coordination team to initiate a records request; most requests are fulfilled within one business day.
